In order to obtain a residence permit, a temporary residence permit or a Residency Registration Certificate you must complete all related formalities and register in person at the regional Voivodeship office --The Malopolski Voivodeship Office Krakow.
The following documents are required to complete the process:
- Application form – to be completed in Polish and is obtainable from the Malopolski Voivodeship Office Krakow.
- A passport – which is valid for a minimum of 3 months longer than the period of time requested in your application form.
- Purpose of stay – a declaration, normally from an employer or an educational institute, stating the total time period of employment or study.
- Declaration of sufficient resources – proof that you can cover your own costs while in Poland.
- Health Insurance documentation – proof that you possess valid health insurance.
- Photos – 2 photos, 4.5x3.5 cm and in colour, are also required.
All documentation and their sworn translations are to be presented in their original. Copies of each document are required. The charge to issue a residence permit is about 30 PLN.
